Bacteriostatic Water: What the Benzyl Alcohol Does
Bacteriostatic water is the least interesting item in most orders and the one most often misunderstood. It is not a grade of purity, not a buffer, and not interchangeable with sterile water. It is water for injection with one additive, and that additive is the entire point.
Bacteriostatic water for injection USP is water plus benzyl alcohol, typically 0.9% (9 mg/mL), added as a bacteriostatic preservative. Label pH is around 5.7. The preservative is what makes repeated withdrawals from one vial defensible. The same preservative is why the label carries a boxed warning against use in neonates, and why the product is not isotonic on its own.
What is actually in bacteriostatic water
Two components. Water for injection, and benzyl alcohol at 0.9% — 9 mg per millilitre — as a bacteriostatic preservative. Some presentations use 1.1% (11 mg/mL). There is no buffer, no salt, no tonicity agent. The label states a pH of 5.7, with a specification range of 4.5 to 7.0.
That mildly acidic pH is worth noting when you are working with peptides, because a peptide’s solubility is governed by how far the solution sits from its isoelectric point. It is a small effect for most sequences and a decisive one for a few.

What the benzyl alcohol is for
Bacteriostatic means inhibiting bacterial growth, not killing bacteria. Benzyl alcohol disrupts membranes well enough to stop organisms proliferating in the vial between punctures. It does not sterilise anything, and it does not undo a contaminated draw.
This is what separates a multiple-dose container from a single-dose one. The official label describes it as a multiple-dose container from which repeated withdrawals may be made — a description that depends entirely on the preservative being present.
Bacteriostatic water versus sterile water
Sterile Water for Injection contains no preservative. It is supplied in single-dose containers and, once the stopper is pierced, there is nothing in the vial discouraging growth. Bacteriostatic water is the same water with benzyl alcohol added.
So the choice is not about quality. It is about whether the vial will be entered once or many times, and whether anything in the work is incompatible with benzyl alcohol. Some protein and peptide preparations are: benzyl alcohol has been reported to promote aggregation in certain protein formulations, which is a real consideration rather than a theoretical one.
- Sterile water: no preservative, single-dose, nothing inhibits growth after piercing
- Bacteriostatic water: 0.9% benzyl alcohol, multiple-dose, growth inhibited between draws
- Neither is isotonic as supplied
- Neither is a buffer — there is no pH control once something is dissolved in it
The neonate warning on the bacteriostatic water label
The official label carries a boxed warning: NOT FOR USE IN NEONATES. The contraindications section is explicit that solutions containing benzyl alcohol must not be used in that population because of the preservative’s potential toxicity.
This is a genuine, historically grounded restriction rather than routine label caution, and it is the clearest illustration of the general principle: the additive that makes the vial reusable is a pharmacologically active substance in its own right, not an inert extra.
It is not isotonic
The label instructs that the product must be made approximately isotonic before use, and warns against intravenous administration unless the osmolar concentration of the additives produces an approximately isotonic admixture. Plain water is hypotonic against cells.
In bench terms: this is a solvent, not a vehicle. Whatever you dissolve in it determines the tonicity of the final solution, and that is your variable to control, not something the water arrives having handled for you.
How long a punctured vial stays usable
The commonly repeated 28-day figure comes from general multiple-dose-vial convention rather than from this particular label, which states no expiry interval after first entry. Treat 28 days as a conservative working convention and the integrity of the stopper as the real limit.
What genuinely shortens a vial’s life is technique: entering it with a used needle, leaving it out of refrigeration, or coring the stopper with a large-gauge needle so it no longer reseals. The preservative slows growth; it does not compensate for any of those.
